We will need the following information available at your appointment in order to address your individual financial situation:
A list of all your debts, including the name and address of each creditor, the account number, and the amount owed. (You can bring the bills themselves, which will contain the information we need.)
All pay stubs for the six-month period before the month of your appointment. For example, if your appointment is July 15, you will need to bring pay stubs from January 1 through July 15. If unable to locate pay stubs you can call your employer for a copy of your earnings history.
If you are self-employed, please call the office for instructions.
A complete copy of your last two (2) years federal income tax return.
We will need to see your driver's license and social security card.
If you are unable to find any of the above items before your appointment, just bring as much information as you can, and we can get the missing items from you at a later time.
If you are married, please bring your spouse to your appointment if possible, even if they are not interested in filing bankruptcy themselves.
